The 49-year-old supermodel and America’s Got Talent judge was dressed as a worm while attending her 21st annual Halloween party at Sake No Hana at Moxy Lower East Side on Monday night (October 31) in New York City.

Heidi‘s husband Tom Kaulitz was dressed as a fisherman who had the worm on his hook as bait!

During the party, Heidi took off her worm costume and walked around in sexy lingerie while keeping the face makeup on. See the pics in the gallery.
