Demi Moore has checked into rehab, according to reports.

The 49-year-old actress has checked into the Cirque Lodge addiction treatment center in Sundance, Utah, according to E!

Demi‘s entrance into the facility comes just a week after she was hospitalized after experiencing seizure-like symptoms during a party at her home.

“She’s on total lockdown and only talking to a small group of people,” a source told E!

Demi recently had to drop out of her cameo as Gloria Steinem in the star-studded film Lovelace and was replaced by Sarah Jessica Parker.

The next day, the 56-year-old actor hosted a 18th birthday bash for daughter Tallulah, who was spotted outside his house wearing a tiara.

Bruce reportedly urged his ex-wife, Demi Moore, to seek help just before she split from Ashton Kutcher.

“He was worried about her and wanted her to get better. Many of her friends did the same thing, but she just wouldn’t listen,” a source told People.

Demi‘s rep previously said that she “has chosen to seek professional assistance to treat her exhaustion and improve her overall health.”

Sarah Jessica Parker will be replacing Demi Moore in Lovelace, her rep confirms to JustJared.com.

The 46-year-old actress will take on the role of famous feminist Gloria Steinem since Demi dropped out after her recent hospitalization, according to EW.

In early January, it was announced that Demi would be taking on the role of Gloria.

SJP will be joining a cast that already includes Amanda Seyfried, Peter Sarsgaard, Adam Brody, Juno Temple, and Wes Bentley.

The 911 call placed from Demi Moore‘s home before she was hospitalized has been released.

“She smoked something. It’s not marijuana, but it’s similar to it. It’s similar to incense. She seems to be having convulsions of some sort,” the female caller says on the tape, according to People. “She’s semi-conscious, barely.”

Another woman can be heard telling the operator, “She’s shaking, convulsing, burning up. I’m taking cold water and putting it on her back because she’s burning up.”

The 49-year-old actress was rushed to the hospital Monday night (January 23). Her rep said Demi had “chosen to seek professional assistance to treat her exhaustion and improve her overall health.”

Demi Moore was slated to play feminist Gloria Steinem but pulled out after her recent hospitalization. Us Weekly reports Mary Louise Parker is stepping in for the role.

Demi Moore is seeking professional assistance after being rushed to the hospital on Monday night (January 23).

Paramedics responded to the 49-year-old actress’s home in L.A. after a 911 call came in at 10:45 p.m. TMZ reports the hospitalization was due to substance abuse.

“Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health,” says her rep.

“She looks forward to getting well and is grateful for the support of her family and friends,” the rep added.
